Your first visit to Orchard Medical Center establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, pertinent family health history, surgical history, allergies and immunizations. This visit is expected to take longer than your typical follow up or sick visit as we work to build your chart and get to know each other.
Please bring your photo I.D., insurance card and most update medical list to your first visit. If you have access to your previous medical records, we ask that you have these forwarded to us PRIOR to your first visit.
